Clinical Observation of Burns in Infancy and Children



Abstract
During the past 8 years from January, 1967 to December, 1974, 349 burned patients were admitted and treated at the surgical department of Kyungpook National University Hospital. Among these patients, 178 patients were under the age of 14. After analysis of these, pediatric burned patients, following results were obtained.
1. Pediatric burned patient occupied 51% of all burned patients.
2. Males were affected more- than females, 109 cases (61.3) in male and 69. cases (38.7%) in female. ,
3. About 82% of pediatric burned patients were under the age of 6, especially 55%" of them were under the age of 3.
4. The incidence of burns was low slightly in the hot season from May to September. 5. Scalding burn was 73.6% and flame burn was 21.9%.
6. All burned patients under the age of 9 injured home indoor, and the home outdoor burns occurred over the age of 10.
7. Most of the cases burned at several parts of the body simultaneously.
8. 78.5% of the cases was between 5 and 30% of the burned surface and all cases involved beyond the 2nd degree burn.
9. The burn wound dressing applied 45 cases with the exposure method and 133 cases with the occlusive method and the topical agents used were 0. 5% silver nitrate solution, nitrofurazone ointment, gentamicin cream and sulfamylon cream etc.
10. By the comparative studies of 0.5% silver nitrate solution-treated group, nitrofurazonetreated group, and gentamicin-treated group,
a) the incidence of the septicemia was 20%, 19% and 12% respectively and least in the gentamicin-treated group.
b) the incidence of wound infection was 27%, 25% and 18% respectively and least in the gentamicin-treated group.
c) the mortality rate was 40%, 29%, and 15%, and highest in the silver nitrate-treated group and lowest in the gentamicin-treated group.
11. The Brooke¢¥s formula and Evan¢¥s formula were employed for the fluid therapy in the most of the burned patients and electrolytes solution used mainly was Hartman¢¥s solution and 1 : 2 : 3 solution.

12. The autograft was performed in 15 cases and the mean day of grafting was between the thirtieth. and the fortieth post-burn days.
13: In 41 cases of the burn wound culture, Pseudomonas aeruginosa(53. 7%), Staphylococcus aureus (17. 1%), E. coli (7.3%), Candida. albicans (2.4%), and the other organisms were . cultured.
14. The complications of the burns were wound infection (24.7%), septicemia (18.0%), respiratory difficulty (14.0%), cicatrical contraction (13.5%), convulsion (12.4%), upper G-I bleeding (10. 7%), etc. in order.
15. The mortality rate was 28.1%, cause of death was septicemia (40.0%), burn shock
(20.0¡Æ%0), pneumonia (10. 0%), airway obstruction (8.4%) in order. In 5 cases of the deaths, its cause was undetermined.
